Mon-Fri 37.5 hours per week

We are recruiting for a leading charity based in the Healthcare sector, for a Hub Coordinator. This post is vital to the delivery of our service contracts and makes an important contribution to the services provided across East Kent. As the first point of contact for individuals providing care, the Hub Coordinator is responsible for ensuring an efficient and effective telephone triage service is provided.

Main Duties Include:
- Provision of a telephone triage, assessment and support service to service providers
-Responding appropriately to calls received in the Hub, directing the caller to the most
appropriate team member or service
-Manage incoming referrals, ensuring procedures are followed and information systems are
updated
-Provision of information to service providers and professionals over the telephone, by e-mail and during visits to the Hub
- Work with individuals to identify their needs and appropriate support options to meet needs
-Building a rapport with individuals and maintain contact using their preferred communication method (including telephone, email, skype, face to face)
-Empower individuals by supporting them to develop confidence and skills to encourage self-
advocacy
 Undertake assessment reviews
 Liaise with and make referrals to other organisations on behalf of individuals
 Assess individuals eligibility to access funded support and grants
 Arrange and coordinate support services for individuals through the correct channels

The successful candidate will have a good understanding of the care industry and the challenges individuals face. Along with this, a background in the NHS or Social care is essential.
