We are looking for a confident & experienced colleague to join the Youth Employability Service (YES) to provide CIAG to 15-25-year-old young people with SEND and / or an EHCP.

The role works closely with the Assessment and Planning Team (within the ISEND department at East Sussex County Council) and the adviser will be expected to build positive working relationships, attend team meetings, relevant training, and input into relevant databases.

The adviser will liaise with allocated school to provide support to young people from year 11 to address barriers, engage with named placements & parent or carers, to improve the young persons’ chance of sustaining post-16 provision in the short and longer term. They will also work closely with post 16 providers to identify young people who are not attending or who may be at risk of withdrawing, to intervene and support them back into education or training.

This post covers the Eastern side of East Sussex and is based in Hastings.

Main Responsibilities:

- Work collaboratively with other East Sussex County Council services to best support a vulnerable cohort, including co-location with the Assessment and Planning Team (ISEND).
- Support the transition of young people with an EHCP from 15 to 18 years in accordance with their relevant plan.
- Support to young people with an EHCP in Year 14 and above as a brief intervention.
- Liaising and communicating with schools to identify students from Year 11 who will benefit from 1:1 transition support and are at risk of NEET.
- Support Year 11 students to ensure they have a September Guarantee and support with their transition from secondary education to post-16 education

Please note this role is fixed term until 30th September 2023 (with the possibility for extension).
