Could you help patients get home?

We’re looking a bright, hardworking and organised individual to join our established team based in Medway Maritime Hospital. You’ll work with adult patients who need extra help when being discharged from hospital, supporting them to navigate through health, social care and community services, think about what best will maintain and improve their independence, health and wellbeing, and make it happen. By making sure the right support in place, people can go home sooner, and the risk of readmission is greatly reduced.

Navigators bring their own experience to the role, so you might have worked in a hospital or other formal setting, completing assessments and support plans, or directly providing social care or health services. Perhaps you have worked or volunteered in a community or medical setting, or have personal experience of accessing services for yourself, a relative or friend.

It’s a really varied role, and you’ll spend time meeting with patients and families. You’ll get to know local services, build networks and work in partnership with health teams and other agencies. You’ll be working in a supportive and friendly team, both in the hospital and with our wider community-based service.
