Carer Support Coordinator

Occasional travel across East Kent may be required with this role

This role is vital to the delivery of the charity's service contracts and makes an important contribution to the range of Carers' services provided across East Kent. Within this role you will primarily be receiving calls from Carer's and professionals within the industry being their first point of contact to offer up to date information within their caring role.

Key Responsibilities include:

.Provision of a telephone triage, assessment and support service to Carers
.Respond appropriately to calls received in the charity's Hub, directing the call to the most appropriate team member
.Manage incoming referrals, ensuring procedures are followed and information systems are updated
.Offering information to Carers and Care professionals via telephone or email
.Work with Carers to identify their needs and appropriate support options available to them
.Visiting of Hospital and healthcare settings to review service available
.Undertake assessment reviews which could include home visits to Carers to assess their telecare and equipment needs
.Attend support groups or events to facilitate sessions for Carers
.Liaise with and make referrals to other organisations on behalf of Carers
.Assess Carer's eligibility to access funded support such as the CCG
.Ensure all organisational policies and procedures are to adhered to including Safeguarding, Confidentiality, Equal Opportunities, data Protection, Health and Safety and lone working

The ideal candidate will have experience of leading Carers or Healthcare professionals and has an understanding of the Health and Social Care sector. You must also have a full UK driving license and access to your own vehicle.
