You'll be part of a small team responsible for the case management of 27 vulnerable young people aged 16–25 with a background of social exclusion. You'll provide support and guidance, enabling people to live independently within the local community. You'll support young people to obtain life skills, education, qualifications, training and employment.
Key tasks include:
• Liaising with Children’s Services, relatives, statutory and voluntary agencies
• Providing housing related support in line with our policies and procedures and the Supporting People Quality Assessment Framework (QAF)
• Quarterly support plan meetings
• Risk assessments
• Regular key work meetings on a case management basis in accordance with Medway Council’s Commissioning Contract obligations
• A focus on education, training and employment outcomes for young people who are not engaging in education, training and employment (NEET)
• Particular emphasis will be placed on providing team around a child (TAC) meetings or team around a young person (TAYP) meetings
• Implementing Resettlement plans
Working Hours:
37 hours per week, Monday to Friday. There’s a two-week shift pattern and you’ll need to be able to work between 8:00am – 9:00pm (There's a half hour unpaid break per day.)
For the role, you'll need:
• Proven experience in working with young people aged 16–25
• Knowledge of housing benefit and personal benefits
• Experience of working with complex support needs
• The ability to be proactive, enthusiasm to interact with young people in a positive way
• To be able to work independently in an open environment
• Excellent IT skills are essential and flexibility to adapt to new systems
