We’re looking for someone with experience of working directly with patients in a hospital or similar setting to join our Social Prescribing team. Based in the busy A&E department of Darent Valley Hospital, you will meet with patients to provide personalised, practical support to them, their families and carers, enabling them to make decisions that will improve their life and health circumstances.

You will independently manage a caseload and new referrals, prioritising need and referring on to specialist services, connecting patients with community services to sustain independent living, health and wellbeing. You’ll be working with people facing complex social and emotional challenges so should be a good listener, have time for people and be confident working within safe boundaries.

In this role you will bridge the gap between primary care professionals and social opportunities, so a knowledge of local services and the ability to build effective networks with cross sector organisations is essential.
